Genesis Healthcare Inc., headquartered in Kennett Square, Pennsylvania, comprises one of the nation's largest post-acute care providers with approximately 400 skilled nursing centers and senior living communities in 30 states nationwide.


Genesis Rehab Services supplies rehabilitation therapy to approximately 1,200 locations in 46 states and the District of Columbia with an additional 75 locations where outpatient therapy is provided in the patient's home and community through the Vitality to You business line.


The Genesis mission to “improve the lives we touch through the delivery of high quality health care and everyday compassion" is the principle guiding each individual clinician and each patient interaction.

Manor Care

HCR ManorCare located in Toledo Ohio. They are in 22 states with over 362 facilities for skilled nursing, outpatient rehabilitation, Memory Care, Home Health and Hospice Care. They were recently acquired by ProMedica and are now the 15th largest healthcare system in the nation.

Brookdale Senior Care 

Brookdale Senior Living Inc. is the leading operator of senior living communities throughout the United States. The Company operates 715 Assisted Living Facilities, 185 Independent Facilities, 456 Alzheimer’s and Dementia Units, and 63 Skilled Nursing Facilities/Rehab s & rehab. Their corporate office is in Brentwood, Tennessee. 

Visiting Angels Living Assistance Services

Visiting Angels is the nation’s leading provider of in-home senior care. Visiting Angels began franchising in 1998 in Philadelphia, PA area. Today, Visiting Angels has more than 600 private duty agencies located in 49 states.

Seasons

Seasons Hospice is a community-based, nonprofit hospice providing hospice care to individuals with a life-limiting or terminal illness since 1996. Most of their patients receive hospice care in their home, wherever that may be. Some reside in nursing homes, others live in assisted living or other residential facilities, while many live in a private residence.


Seasons Hospice provides hospice care in 19 states and in over 200 counties. They are one of the top 5 hospice companies in the nation.

Paradigm’s post-acute care services encompass a nationwide discounted network of care providers specializing in long-term acute care, in-patient rehabilitation, assisted living, and skilled nursing facilities.


Assisted living and after-hospital care decisions are often confusing, and it is difficult to determine a facility with the best quality of care, quality of life, and affordability. As everyone has unique care needs, our case coordinators are trained to refer patients to the appropriate facility based on the nature and extent of the individuals’ needs, as well as the specific capabilities of the facility.

Network Coverage – The Paradigm post-acute care contracted network consists of 8,000+ providers/locations in 49 jurisdictions. Paradigm negotiates with providers needed outside of their contracted network. Every nursing facility is credentialed to ensure that licensure and insurance requirements are met and maintained. Further, each facility is Medicare rated at 3 stars or higher.
